Information for new patients
If you are a new patient, prior to your appointment, you could print off the new patient medical history and consent form before you attend an appointment and fill it in with all the necessary information using your own pen in the comfort of your own home.
Please bring it with you to your appointment and don’t forget your face covering. Important in the Covid-19 time.
